Giving a sugar pill to everyone who thought they might have had Covid would result in a 50 percent positive result, as do all studies using a placebo, instead of the actual drug.
A drug or vaccine has to pass that 50 percent efficacy threshold in order to be called effective, otherwise it is the same as a placebo, only with lots of negative side effects.
